Pre-Registration

Preregistration for all courses is necessary. Applications for registration will be processed in the order received, and the Office of Continuing Dental Education reserves the right to end registration when the enrollment limit has been reached. The registration fee must accompany the application.

Registration Fees

 

All registration fees include tuition, course materials, meals and refreshments (when applicable). See individual course descriptions for the tuition fees. The registration fee must accompany the registration form. One check or company credit card may cover several registrations. Please clearly state all participants’ names and the courses for which they are registering.

Handouts, Breaks and Meals

The price of your course includes handout materials and coffee breaks, unless otherwise noted. All-day courses include lunch.  If you have special dietary needs, please call 205.934.3753 at least five business days before the start of the course to inform us of your dietary requirements.

Confirmations

Confirmations will be sent to participants approximately two weeks before the program. For registrations received less than two weeks prior to the program, confirmations will be faxed to those participants who include a fax number on their registration forms.

On-Site Check-In

Please arrive 30 minutes prior to the start of the course to check in and pick up packets. Registration confirmations and signs in the Dental School Lobby will list room numbers for the day’s courses. Same day walk-in enrollments will be accommodated on a space-available basis.

Cancellation Policy

UAB Continuing Dental Education reserves the right to cancel any course, limit enrollment, modify the announced course content, or change the location, time, date, and faculty of any course as may be deemed necessary or advisable.

We encourage you to register early. This will enable us to protect course enrollment and avoid an unnecessary cancellation of a course. In the event that the School of Dentistry feels it is necessary to cancel a program, a full refund of the tuition will be granted. Attempts to contact all registrants will be made at the time cancellation is determined.

UAB Continuing Dental Education Programs cannot be responsible for the refund of any part of the tuition, costs of travel, or lodging as a result of unforeseen circumstances or events beyond its control.

Refund Policy

Tuition must be paid in full at the time of registration unless otherwise noted. We accept Visa, MasterCard, and Discover. Tuition for no-shows will be forfeited, no exceptions.

If you must cancel your registration, please notify the Continuing Dental Education Department as soon as you determine you are unable to attend. Full refunds of tuition are granted only if written notification is received seven (7) business days prior to a lecture course and fourteen (14) business days prior to a participation or limited attendance course. Cancellations received within 72 hours prior to the start of the course will be subject to a 25% administrative fee. The remainder of the tuition may be applied to a future course. Please allow up to 4-6 weeks for some refunds to be processed.
